To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Submit your completed form or letter to USDA by: (1) mail: U.S. Department of Agriculture, Office of the Assistant Secretary for Civil Rights, 1400 Independence Avenue, SW, Washington, D.C. 20250-9410; (2) fax: (202) 690-7442; or (3) email: program.intake@usda.gov.

  • Howard A. Hanlon Elementary Principal Mrs. Alisha Tolentino grew up in Canisteo, NY. She earned a bachelor’s degree from Niagara University, a master’s degree in TESOL from Nazareth College and a Certificate in School Building and School District Leadership from SUNY Brockport. Before coming to O-M, Mrs. Tolentino was an assistant principal, dean of students, and special education teacher throughout the region. Outside of school, Mrs. Tolentino enjoys spending time with her family. With a deep commitment to fostering an environment where every child feels valued and inspired, Mrs. Tolentino looks forward to working closely with our dedicated teachers, staff, parents/guardians, and community members. Her goal is to build on the strong foundation already in place and to ensure that each student not only excels academically but also develops a love for learning and a sense of belonging.
